Direct Primary Care:
$150/mo
** discounts available for additional household members and quarterly or annual payments
Our premium care option to circumvent the requirements and inconveniences typical of traditional insurance and Medicaid plans. We are proud to offer this robust comprehensive membership for a monthly price that is comparable to a single doctor visit.
By eliminating the requirements of insurance, we are able to provide unlimited care when and how you, the patient, wants and needs it. Longer or more frequent visits, phone calls and telemedicine, texting/emailing the provider directly are all included. Access is equally as important as quality.
**Pricing begins at $150/month.
2nd and 3rd household members can be added to the plan for a discounted price of $125/mo per person.
Additional household members can be added for a discounted price of $100/month
Additional rated discounts are offered for upfront payments on a quarterly semi-annual or annual basis:
-Paying 3 months at once (quarterly payment plan) 10% discount on all plans.
-Paying 6 months ahead (semi annual payment plan) 15% discount on all plans.
- Paying 12 months ahead (annual payment plan) 20% discount on all plans.
HIV management ONLY:
$40/mo
Due to the lack of affordable quality HIV management in the area, we offer this plan for individuals who want just their HIV/ medications. This plan does not establish a physician/patient relationship and it is the individual's responsibility to find primary care elsewhere or to upgrade with our clinic to the Direct Primary Care Plan which would cover all primary care as well as HIV management.
Medicaid Medicare and all Insurance Plans-not accepted
In order provide timely, quality care, we do not accept these forms of payment.
*Medicaid includes AHCCCS, Health Choice, Care 1st, DDD, Mercy Care
*Commercial plans include BCBS United, Ect.
* Medicare includes Humana, Wellcare, ect.
